Statement of consent to personal data processing
I hereby consent to the processing by Narodowy Bank Polski with its seat in Warsaw at ul. Świętokrzyska 11/21, 00-916 Warsaw of my personal data contained in the registration form for the purpose of, and to the extent necessary for, organizing NBP workshop/seminar/webinar/study visit/internship. *
I hereby consent to the disclosure to other members of NBP workshop/seminar/webinar of personal data including name, surname, position, institution, email address for the purpose of the holding of NBP workshop/seminar/webinar. I authorise the publication of any pictures of me taken during the workshop/seminar/webinar/study visit/internship in the NBP informative materials and on the website of NBP.
Information clause
In accordance with Article 13 section 1 and section 2 of the Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation) (O.J.UE.L.2016.119.1), hereinafter referred to as GDPR, we would like to inform you that:
Your personal data controller is Narodowy Bank Polski with its registered office in Warsaw at the following address: ul. Świętokrzyska 11/21, 00-919 Warszawa.

Your personal data will be used for the purpose of processing your application to the workshop/seminar/webinar/study visit/internship.
Your data will be processed on the basis of your consent, i.e. pursuant to Article 6 (1) (a) of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation, “GDPR”).
Your personal data will not be used for automated individual decision-making, including profiling.
Your personal data are transferred to entities providing IT support to Narodowy Bank Polski; the transfer of data is performed pursuant to the concluded personal data processing agreements, and the processing entities process them exclusively at the request of NBP, i.e. they are not authorized to process such data for their own purposes.
Your personal data will be communicated to the hotels chosen as a place of accommodation and selected providers of services (eg. software vendors, catering etc.) for participants for each NBP workshop/seminar/webinar/study visit/internship.
Your personal data will be kept for a period of five years starting from the end of NBP workshop/seminar/webinar/study visit/internship or until you revoke your consent to data processing.
You have the right to request access to your personal data, their rectification, erasure or limitation of the processing. You do not have the right to data portability referred to in Article 20 of the GDPR.
If the basis for processing the data is consent, it may be withdrawn at any time, but the withdrawal of consent does not affect the lawfulness of the processing of data which was made on the basis of consent before its withdrawal.
The provision of personal data is voluntary, yet refusal to provide them will make it impossible for you to register at NBP workshop/seminar/webinar/study visit/internship.
You have the right to lodge a complaint with the President of the Office for the Protection of Personal Data when you consider that the processing of your personal data is in violation of the current legislation on the protection of personal data.
